Beach Dental Care Patient Forms

Discover our patient forms on this page. We recommend you print, fill them out, and bring them to your next appointment with us at Beach Dental Care.

Read through our frequently asked questions below to learn more about our dental office. If you do not see your question addressed here, feel free to contact us!

We are at 1989 Queen Street East, in the white house next to Kew Gardens.

We accept most dental insurance plans. We can bill directly to your insurance. You are responsible for the amount not covered by your insurance.

The Ontario Dental Association sends out a suggestive fee guide for dentists. Our fees are based on the fee guide, and we do not go above their fees. Insurance companies use the same fee guide to decide which level of reimbursement it provides for their plans.

Crowns and bridges, partial dentures, full dentals, and implants need predetermination prior to treatment. This means we send your insurance the estimate with your x-rays, and they let you know prior to treatment how much they will cover. They need to approve the work before it is done because this is considered major work by the insurance companies. Processes such as examinations, cleanings, fillings, and root canals are normally considered basic and do not need predetermination. You need to be aware of the dollar limit your insurance plan will cover per year and at what percentage, as well as what year of the fee guide your insurance company is using. If you have any questions, the secretary can help you. Some insurance companies let us call on your behalf. Others will only speak to you.

During your first visit, we will ask you to fill out a medical and dental history form. We want to understand what you are looking for. If you have not had a complete examination done in the past three years, we will do one if you agree. If you have not had x-rays done in the past three years, we will also take X-rays. If you have had them done, we will ask you if we can get them from your previous dentist.

Nervous Patients: We understand some patients may feel nervous, uncomfortable, embarrassed, or afraid of visiting the dentist. We are proud that you took the first step towards your dental health! Your comfort is our priority and we will do everything possible to put you at ease, which includes promoting a family friendly atmosphere to help you feel better.
